Terms & Conditions

• I agree that the information listed on my Employment Ontario Participation Registration form is true.
• I have read, understand and agree to the notice of Collection and Consent, as listed on my Employment Ontario Participant registration form.
• I acknowledge that my Service Provider has explained its use and disclosure of my personal information for its purpose as listed in the Collection and Consent section of my Employment Ontario Participation Registration form.
• I give consent to the Ministry to indirectly collect, use and disclose my personal information for the purpose set out in the Collection and Consent section of my Employment Ontario Participant Registration form.
• I authorize my Employment Consultant to obtain, release and or exchange any pertinent information with regards to my involvement with Lambton College Community Employment Services with the agencies discussed with my Employment Consultant.
• I understand that Employment Ontario Service Providers deliver Employment Ontario programs and services under an agreement with the Ministry of Labour, Immigration, Training and Skills Development and the City of Windsor.
• I am providing my electronic signature and consent to participate in the full suite of programs and services offered by Lambton College Community Employment Services.

CES Program Services Agreement

• I understand that Community Employment Services is not a placement agency and that they work collaboratively with me to find sustainable employment.
• I give consent to Lambton College Community Employment Services to send Job Alerts and other employment related program and service information, via email or text message to the contact information provided in the EO Participant Registration form (you may withdraw your consent at any time by indicating that you wish to unsubscribe).
• I agree to continue to self-market for employment and regularly keep my Employment Consultant informed of that status of my job search applications and employment outcomes.
• I agree to notify my Employment Consultant if/when I find employment and provide details in regards to this employment.
• I commit to participating in and following through with the activities within the Employment Service plan developed with my Employment Consultant.
• I understand that an Employment Consultant will be in contact with me after my file has closed for regular follow-up.
